Job Summary:

• Evaluate patient nutrition needs by obtaining nutritional/lifestyle history and performing nutritional physical exams. 
• Participate in the delivery of a uniform nutritional/lifestyle program as prescribed by applicable physician/clinician staff
• Provide nutritional counseling and perform Endo-PAT and ECG testing.
• Establish rapport with each patient using a personalized medicine approach to unique patient profile while meeting them where they are in abilities to making healthy changes.
• Provide a brief overview of the patient’s foundational program being based on a prescribed food plan, select medical foods and dietary supplements including lifestyle factors which include physical activity, stress management, sleep hygiene and water intake, etc
• Discuss, initial daily food and beverage log and the importance of journaling in food and beverage log to build awareness, promote self-discovery and compliance in relation to food.
• Perform the Bioelectric Impedance Analysis (BIA) test and explain BIA basics to patient. 
• Complete EMR progress notes on patient chart indicating patient evaluation, treatment and status in a timely manner.
• Develop/Update Nutritional and Lifestyle Materials for Group Medical Visits and Behavioral Change Support Groups
• Conduct actual and virtual group counseling and Q&A sessions within context of Group Medical Visits and Behavioral Change Support Groups
• Participates in provider monthly meetings and weekly education meetings
• Perform patient intake and review of medical history and completion of vital signs/anthropometric measurements.
• Maintain confidentiality regarding all patient matters and patient data as per HIPAA regulations.
