• Answer, engage, and assess member calls related to Behavioral Health and/or Substance Use Disorder

• Accept referrals from hospital discharge planner after they have identified patient agency choice and type of home service needed.

• Performs on-site and off-site review of patients in designated facilities to assist in identifying patients who could clinically benefit from acute inpatient rehabilitation services.
• Performs on-site and off-site clinical evaluations to assist the Acute Rehabilitation Medical Director in determining appropriateness for admission.
• Provides written materials designed for the Acute Inpatient Rehabilitation Unit to Case Coordinators to share with patients and family members.
