Case Coordinator Job Description Template
1. About the Role
Coordinating a caseload means knowing when a student's attendance pattern signals a crisis before anyone else does. This role owns the day-to-day management of participant services - from HIPAA/FERPA-governed documentation and forensic-level assessments to discharge planning and referrals across county probation, child welfare, and community health systems. No two cases follow the same path. Case Coordinators move between family home visits, multi-disciplinary staffing meetings, and real-time crisis response, holding the thread of continuity for participants who depend on a single point of accountability.
2. Position Summary
As the Case Coordinator, you maintain service continuity for individuals and families navigating human services programs, translating complex system requirements into actionable care plans that measurably improve participant outcomes. You operate within an interdisciplinary team - typically reporting to a program supervisor or clinical lead - carrying a defined caseload across settings that include schools, homes, courts, and community agencies.
3. Why Join Us
Career Impact: Hands-on experience across child welfare, juvenile justice, and behavioral health systems builds specialized expertise that commands recognition among county agencies, nonprofits, and multidisciplinary clinical teams.
Business Impact: Families and program participants depend on this coordinator to navigate eligibility, prevent service gaps, and keep discharge plans on track - gaps in this work translate directly into missed interventions and program non-compliance.
Growth Opportunity: The caseload exposure and cross-system fluency developed here create a direct path toward senior case management, clinical supervision, or program coordinator roles within human services organizations.

- Document all case contacts, assessment findings, and service updates in compliance with HIPAA, FERPA, and program-specific reporting standards.
- Conduct or support psychosocial assessments, intake interviews, and risk screenings to inform care planning decisions.
- Monitor participant attendance, behavioral indicators, and service utilization, escalating concerns to supervisors and adjusting plans accordingly.
- Participate in staffing meetings, school conferences, and court-related proceedings as a representative of the participant's service team.
- Support program compliance by completing grant-required documentation, data collection, and outcome reporting within established deadlines.
5. Required Qualifications
