Job Summary:

• Provides physical therapy,  per physician order, including initial evaluations, which may include start of care PT only cases, range of motion exercises, chart documenting, and filing.
• Coordinates appointments.
• Applies rehabilitation devices, such as hand rolls, slings, braces, and sets up walkers, and arranges for canes.
• Establishes times and performs gait training with patients, and ambulates patients.
• Maintains and documents patients' charts, restorative needs, and potential.
• Provides input into inter-disciplinary team meetings and implements recommendations with staff.
• Ensures appropriate policy/procedure is followed.
• Completes 5-6 re-visits or a PT open and 2-3 re-visits per eight-hour day, including completion of all paperwork.
