Qualifications: BS in Health and Wellness with 3 years of Experience

• Possess a NAC or HCA (Home Care Aide) license and requirements for TB, CPR/First Aid
• Recognize and report adverse changes to RN/LPN - Wellness Nurse (campus) or Home Health office (after hours for Wellness Nurse and for all off-campus clients).
• Be able to read and write in English and to communicate with clients/patients/staff/coworkers/agency staff.
• Be able to remain calm under stress.
• Ability to coordinate multiple tasks simultaneously.
• Ability to understand and respond to a diverse population.
• Ability to give all direct client care within the scope of license and according to the written Plan of Care.
• Ability to move about in congested areas.

Qualifications: BA in Community Health with 5 years of Experience

• Possess and maintain a current Home Health Aide certification and Certified Nursing Assistant (CNA) certification
• Possess and maintain a current Basic Life Support (BLS) certificate.
• Ability to work competently in the use of Electronic Medical Record (EMR).
• Be able to work as a team and collaborate with clinicians.
• Previous home health or hospice experience
• Second language skills in Spanish
• Be able to pass a basic written competency and skills test
• Demonstrates strong work ethic and a commitment to providing exceptional caregiving services.
• Ability to maintain excellent interpersonal and cooperative relationships

Qualifications: BS in Rehabilitation Services with 2 years of Experience

• Experience as a Home Health Aide in a Home Health Agency 
• Possess California Home Health Aide Certification
• Ability to work effectively with the public.
• Ability to plan daily patient schedule efficiently.
• Ability to use standard Home Health Nursing supplies and equipment.
• Ability to read, write, speak, and understand English and communicate effectively
• Be able to self-direct with the ability to work with little direct supervision
• Ability to treat clients, staff, and the public with courtesy, respect, and present a positive public image
• Demonstrates a strong work ethic and a positive team attitude.
• Be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. 

Qualifications: BA in Liberal Arts with 1 year of Experience

• Ability to read, write, and perform arithmetic calculations
• Possess home care experience
• Possess CNA (Certified Nursing Assistant), GNA (Geriatric Nursing Assistant),  HHA (Home Health Aide) certification, and CPR certification. 
• Completion of MedStar Health's home health aide wound, diabetes, and CHF competencies
• Verbal and written communication skills.
• Possesses basic computer skills.
• Working knowledge of how to cook/prepare meals, special diets, and diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses. 
• Ability to maintain a professional appearance and professional communication behaviors in patient homes with both the patients and their family members/caregivers. 
• Able to competently take temperatures, heart rates/pulses, and respiratory rates, and identify changes in patient condition.

Qualifications: BS in Exercise Science with 4 years of Experience

• Experience working in a facility and a similar homecare agency
• Strong communicator verbally & in writing using the English language
• Have familiarity with memory care
• Experienced Certified Home Health Aide who supports professional nursing staff and other healthcare disciplines.
• Demonstrate good judgment and skill in reporting information, and demonstrate knowledge of developmental stages. 
• Ability to provide education to meet certification, license, or registration requirements.
• Effective communication skills, such as verbal and written
• Ability to prioritize and think critically, including scheduling and preparing for visits
• Organizational skills, time management, including scheduling and preparing for visits

Qualifications: BA in Behavioral Science with 1 year of Experience

• Current CPR BLS-Health Care Provider.
• Prior home care experience 
• Demonstrates knowledge of CAN skills as they relate to basic patient care.
• Demonstrates mature, responsible behavior and understands the need for patient confidentiality.
• Must be able to read, write, and speak English.
• Must be able to follow directions and accurately report to the Program Manager, Registered Nurse, or appropriate designee any changes in client condition.
• Must have available, reliable transportation to and from assignments.
• Must be able to effectively communicate with RN case managers
• Must be a self-starter and goal-oriented

Qualifications: BS in Nutrition with 2 years of Experience

• Ability to read and write proficiently in English
• Be able to provide reliable transportation.
• Completed the Direct Support Professional curriculum as adopted by DHHS, DHHS’s approved Assessment of Prior Learning, before July 1, 2011, or has completed the Direct Support classes. 
• Completed the Introduction to Developmental Disabilities, Professionalism, Individual Rights and Choice, and Maltreatment modules.
• Be able to provide Individual or Work Support Group, must have completed the additional employment modules from the Direct Support classes, to provide services.
• Be able to provide Career Planning, must have completed the additional employment modules from the Direct Support classes, and an additional 6 hours of Career Planning and Discovery training provided through the Workforce Development System.
• Successful completion of a formal nursing assistant certification (CNA) training program and/or a written skills test and competency evaluation.
• Experience working in Medicare Skilled Home Health Care
• Be able to visit patients in private homes and senior living communities comfortably

Qualifications: BA in Aging Studies with 3 years of Experience

• Successful completion of a state-approved Home Health Aide course.
• Experience as a Home Health Aide
• Be able to be listed in good standing on the New York State Nurse Aide Registry
• Meets passing criteria on the DEARS Ethics test
• Ability to read, write, speak, and understand English
• Must be able to follow verbal and written instructions and document services provided
• Have personal care experience and have a valid CNA license
• Must be able to function independently and maintain satisfactory relationships with administrative staff, clients, and client family members
• Ability to read and understand task assignments, ability to report verbally, and provide written documentation
