Job Summary:

• Provides nursing care to patients with diabetes within the Community Program (General Internal Medicine)
• Utilizing clinical judgment, participates in interprofessional Diabetes clinics to assess current health status, interpret diagnostic test results, provide education and support to patients with diabetes (Type 1 and Type 2). 
• Communicates significant clinical findings to attending physicians
• Assesses and teaches patients with diabetes on GIM in-patient units re diabetes self-management (blood glucose monitoring, insulin admin, etc)
• Promotes continuity of care by participating in discharge planning for patients with diabetes on GIM in-patient units to ensure appropriate and timely referral to outpatient supports (SUNDEC) (Chronic Disease Self-Management Programs) etc. and to reduce length of stay and readmission rates
• Consults with patient/family and interprofessional team members to develop and revise comprehensive care plans for patients with diabetes
• Work with medical directives and adjust insulin as appropriate for patients with diabetes
• Advocate on behalf of patients with diabetes within the team, program, organization and community
• Act as a resource for other nurses, health care professionals and learners around diabetes care
• Liaise with interprofessional team members as appropriate.
• Attends SUNDEC team meetings and participates in self-directed learning activities

Job Summary:

• Responsible for the daily smooth running of the Diabetes Care Centre.
• Provide diabetes education and consultation to individuals of all ages, groups and the community.
• Offer professional guidance and assistance to caregivers, to help them cope caring for someone with diabetes.
• Responsible for all sales in the Diabetes Care Centre.
• Source for appropriate updated diabetes information, pamphlets and posters for the Centre.
• Direct and guide patient teaching and all diabetes activities for the nursing staff, and are able to maintain high standard of professional diabetes care.
• Educate/ train GKL nurses so that they are proficient in the management of diabetes care.
• Organize and coordinate all diabetes public forums.
• Be able to negotiate pricing of glucose monitoring devices, test strips lancets and other diabetes products.
• Be a resource personal for all public campaigns organized by GKL.

Job Summary:

• Provide patient care to patients diagnosed with diabetes in pregnancy or requiring pre-conceptual counselling.
• Assist the Program Director in the monitoring and assessment of nursing care, ensuring standards of practice are met.
• Evaluate care given via direct observation, review/audit of relevant patient documentation
• Evaluate care given via consultation with co-workers and other health care professionals and interviews with patients and families.
• Maintain good public relations with patients, relatives and staff.
• Update one knowledge continuously by attending in-service and updated diabetes education.
• Undertake any related duties as might be required, from time to time.
• Perform any additional duties as assigned by the immediate superior.
• Responsible to be engaged in the department and Hospital quality indicators.
• Provide comprehensive, evidenced based specialist diabetes clinical care, training and consultancy to both people with diabetes and other healthcare professionals. 
• Be providing care in a range of settings including the community clinic centres, patients home and nursing homes. 
• Working with primary and secondary care practitioners to deliver excellent care for patients through procedures, policies, reporting and governance arrangements and covering education programs for both patients and healthcare professionals.
