• Provides spiritual care that meets the spiritual and emotional needs of patients, patients' families, staff, providers, and volunteers.

Job Summary: 

• Take responsibility for carrying out clinical spiritual care activities
• Communicate concerns and/or needs to the Director of Spiritual Care and Clinical Pastoral Education. 
• Actively participate in clinical learning opportunities, report learning through clinical presentations, reflections, syllabus course requirements of CPE, etc.
• Assess patient needs for spiritual care and counsel and initiate or recommend programs and services to meet those needs. 
• Lead patients in regularly scheduled spiritual services and chapel functions or arrange for visiting clergy to do so. 
• Provide rituals and perform religious rites, e.g. baptism, sacraments, honor walk, blessings, etc., or facilitate appropriate religious leaders to do so.

Job Summary: 

• Participate in interdisciplinary team meetings/rounds and activities in the assigned clinical areas.
• Complete the Spiritual Care template in OneChart that includes a Spiritual Assessment, Intervention, Outcomes and Plan, and AIOP for every patient.
• Attend spiritual care consults regularly and follow up on non-urgent consults within 2 hours and respond to urgent requests immediately.
• Provide chaplaincy service to all self-harm patients as assigned, ensuring comprehensive follow-up.
• Continually be available for chaplaincy staff support to all employees, and the hospital community, offering individual and group opportunities as well as services and rituals as appropriate.
• Be available for consultation with hospital staff and other individuals on matters about hospital ministry.

• Provides pastoral care to patients and their families, including spiritual assessment, sacramental ministry, bereavement assessment and support, bio-ethics or health care decision-making support, consultation with staff, and connection with the religious community.

Job Summary: 

• Participates in supervised educational activities congruent with the needs of the spiritual care department, the facility and the PSA under the direction of the CPE Supervisor and in conformance with the standards and outcomes of the Association for Clinical Pastoral Education (ACPE).
• Provides spiritual care for patients, families and team members in assigned areas, including prayer, sacraments, and religious, spiritual or emotional support.
