Job Summary: 

• Works collaboratively with clinical teams in managing Cardiac Surgery patients in the outpatient environments. 
• Collects data pertinent to the patient’s health and situation including interpreting diagnostic tests and procedures relevant to the patient’s status
• Under the guidance of the Structural Heart Medical Director, coordinates and executes all steps in the planning, preparation, care and follow-up of the structural heart patient to expand the scope and volume of the structural heart program
• Compares and contrasts clinical findings, and normal and abnormal variations in formulating a diagnosis.
• Provides direct patient management and patient care including pre-procedure assessments, Histories, and Physicals, ordering and interpretation of diagnostic tests/studies, post-procedure management, assistance with discharge, post-hospitalization management, education, and counseling
• Cares directly for clients and helps manage complex patients. 
• Promotes patient engagement by explaining the treatment plan, and identifying and addressing any expected obstacles.
• Participates in the quality and patient satisfaction review process, incorporating feedback into improvement plans and associated quality forums. 
• Uses quality data to enhance the effectiveness of care delivery.
• Uses current healthcare research findings and other evidence to expand clinical knowledge, enhance role performance, and increase knowledge of professional issues
• Partners with the interventional cardiologists, cardiac surgical team, and other clinical staff to coordinate care for the structural heart patient from referral through pre-procedure testing, procedure planning, post-procedure management, and follow-up
